Prior to phase 2 cardiac rehabilitation, treadmill exercise stress testing (TET) is usually done to determine risk stratification and functional capacity of patients with coronary artery disease. However, at the Philippine General Hospital, the subsidized rate of having the test done is still unaffordable to most of its indigent patients. Given its prohibitive cost to economically challenged patients, should TET still be done? Can the low cost 6-Minute Walk Test (6'WT) be constituted for the TET? Twenty-five adult patients referred and cleared or possible cardiac rehabilitation phase 2 were recruited and all were made to undergo both the treadmill test and the walk test. Cardiovascular parameters were recorded during peak of treadmill testing and the immediately after the walk test. The 6"WT yielded the following results: mean systolic pressure (SBP)= 132 mmHg, mean diatolic pressure (DBP)= 83mmHg, mean arterial blood pressure (MABP)= 118.32 mmHg, mean heart rate (HR)= 87.64 beas/min., mean METS achieved=2.55; while the treadmill test showed SBP=164 mmHg, mean DBP= 94 mmHg, MABP= 142 mmHg, meanHR= 133 beats/min., mean METS achieved= 8.26. Using paired t-tests, at significance level of p < 0.05, results revealed significant differences between the means of systolic, diastolic and mean arterial BP, heart rate, and METS taken from both exams. The results of this study show that the cardiovascular responses to the 6'WT are of lower magnitude as compared to those resulting from the treadmill exerciest test. This implies that care should be taken when using the 6'WT because it may underestimate the true functional capacity of patients who did not undergo treadmill exercise stress testing.
